on the patients attending Out-patient Therapeutic Program (OTP) 

Nine Indicators are identified to be collected;
 -Application Name 
	e.g: otp
 -Location (CODE) of the OTP Site the reporter is reporting from
    e.g: WAD - for Waddesa
 -New Admission: 
		Number of new OTP patients accepted for the reporting
        period
 -Cured:
 -Died:
 -Defaulted:
 -Non-Responded:
 -Transfered to other sites (OTP/TFU):
 -Medical Transfer:
